This single online module is for learners in any role involved in promoting advance care planning (ACP).
Previously available only within the complete Online ACP Facilitator Curriculum, it is now available as a stand-alone module for a broader audience.
Through example scenarios and clips of facilitated ACP conversations, this module builds an understanding of the purpose of ACP and the role of a facilitated conversation, thus preparing all learners to help affirm the importance of ACP. Content demonstrating the general communication skills will benefit the entire team as these are foundational to all person-centered interactions.
Order includes a Communication Skills pocket card and an ACP Introduction Guide for each learner.
Educational Format
Learner-directed online course, including narration, recommended supplemental reading materials, interactive activities within the module itself (e.g., click and drag, matching), case examples, post-test, and course evaluation.
Media
Interactive, learner directed audio with graphics.
